This article addresses the conflict between the right to life, health, and religious freedom of Jehovah's Witnesses and the need for blood transfusions in emergency situations. It identifies as a problem the lack of clear legal guidelines for healthcare personnel when faced with a refusal of transfusion and the life-threatening risk that this entails. The objective of the study is to analyse the Ecuadorian legal framework in contrast to other legal systems, with the aim of proposing criteria to guide medical practice. The methodology used is qualitative, with a descriptive-analytical documentary approach. The results indicate that, in Ecuador, as in many countries, the rights to life, health and religious freedom are protected and guaranteed; however, there remains a regulatory vacuum and a diversity of positions in favour of life or patient autonomy.
